Tariff of reimbursement of costs for disclosure of information according to § 5 par. 1 letter f) of the Act on Free Access to Information

1. Reimbursement of costs associated with making a copy and sending the information:

   • Making a copy
      - 1 page, format A4 - € 0,10
      - 1 page, format A4, double-sided printing - € 0,15
      - 1 page, format A3 - € 0,15
      - 1 page, format A3, double-sided printing – € 0,30

   • Envelopes
      - envelope, format A6 - € 0,10
      - envelope, format A5 - € 0,20
      - envelope, format A4 - € 0,30
      - envelope with bottom - € 0,50

   • Provision of information on technical data carriers CD, DVD - 1 pc - € 1,00
     Material costs associated with sending information are determined according to the current price list of Slovenská pošta, a.s.

2. Information sent by e-mail and telephone is provided free of charge.

3. According to the Decree of the Ministry of Finance of the Slovak Republic no. 481/2000 Coll. on the details of reimbursement of costs for disclosure of information, the reimbursement of costs shall be determined as the sum of the costs of the Tariff.
   The applicant can pay them to account no. SK80 8180 0000 0070 0016 6213, or by direct payment in cash at the treasury of the Armed Forces Academy.

Each request for information must have the prescribed requirements specified in § 14 par. 2 of the Act on Free Access to Information.

It must be clear from the application to whom the liable person is addressed, the first name, surname, name or business name of the applicant, his/her residence address or registered office address, the information to which the application relates and what method of disclosure the applicant proposes.
